New Patients

Your First Consultation

To make an appointment, you must have a referral letter from your doctor. Appointments can be arranged by telephoning (03) 9874 8200.

At your first appointment you will be asked to complete a new patient registration form. This   New Patient Registration Form can be downloaded, and you are welcome to complete it ahead of time and bring it with you to your first appointment.

What To Bring

Please remember to bring your referral letter along with all other information supplied by your referring doctor, including reports, x-rays and CT or MRI scans. If you have not had any X-rays prior to your appointment, or if your X-rays are older than 6 months, please let us know in advance. It is often worthwhile and timesaving to have up to date X-rays prior to your appointment, and this can be facilitated if required. Also, bring any information relevant to your medical history, such as current medication, allergies and any ongoing medical problems.

When To Arrive

It is best to arrive about 15 minutes prior to your appointment so that you have sufficient time to complete paperwork such as your   New Patient Registration Form that includes a brief health questionnaire and a privacy policy. We encourage you to download and complete this form ahead of time as this will help save some time for you on the day.
